In the historic center of the village, the church is also known as the Chiesa della Compagnia di San Galgano.

Of the San Sebastiano Church in Chiusdino, currently home to the Inclita ed Insigne Compagnia di San Galgano, the year of construction is unknown. Yet, the building’s wall structure declares a 15ᵗʰ-century origin, probably related to the need to ward off epidemics and plagues, calamities against which Saint Sebastian was traditionally invoked.

On the façade of the church are two plaques: one depicts Saint Galgano in the act of plunging his sword into the summit of Montesiepi, attributed to Urbano da Cortona, while the other bears the date 1466. Both come from Siena and were originally placed on the façade of the Monastery of Santa Maria Maddalena belonging to the Cistercian monks of San Galgano.
